认知障碍影响髋部骨折术后再次手术的风险: 挪威髋部骨折登记处报告的 87,573 例手术结果。

Abstract:

: 背景和目的-约 4分之1 的髋部骨折患者存在认知障碍。我们根据挪威髋部骨折登记处 (NHFR) 的数据，调查了患者的认知功能是否影响髋部骨折后的手术治疗、再次手术风险和死亡率。患者和方法-这项前瞻性队列研究包括 87,573-2005 年度 NHFR 报告的 2017 例髋部骨折。使用校正性别、年龄、 ASA 分级、骨折类型和手术方法的 Cox 回归计算再次手术风险和死亡率的风险比 (HRRs)。结果-27% 的患者报告了认知障碍。他们年龄较大 (86 岁 vs. 82 岁)，ASA 分级高于非受损患者。骨折类型和手术方式无差异。认知障碍患者总体再手术率较低 (4.7% vs. 8.9%，HRR 0.71; 95% CI 0.66-0.76) 和接骨术后再次手术的风险 (HRR 0.58; CI 0.53-0.63) 低于非受损患者。认知受损的髋部骨折患者在半髋关节置换术后再次手术风险增加 (HRR 1.2; CI 1.1-1.4)，主要是由于脱位 (1.5% vs. 1.0%，HRR 1.7; CI 1.3-2.1)。后路手术后脱位风险特别高 (4.7% vs. 2.8%，HRR 1.8; CI 1.2-2.7)。此外，由于非骨水泥半髋关节置换术后假体周围骨折，他们再次手术的风险较高 (HRR 1.6; CI 1.0-2.6)。认知障碍髋部骨折患者的 1 年死亡率高于无认知障碍患者 (38% vs. 16%，HRR 2.1; CI 2.1-2.2)。解读-我们的研究结果支持给予认知障碍患者与非障碍患者相同的手术治疗。但由于认知障碍患者半假体脱位和假体周围骨折的风险较高，他们可能不应该进行后路手术或非骨水泥植入物。
